General Information
Portuguese nationals should register at the nearest Portuguese Consulate. For those residing outside of the Metropolitan area who hold valid Portuguese Identification Cards, the registration may be done by mail. Applicants who are not holders of valid I.Ds., must  refer to the instructions on the attached page (see No. 4 below.)

1.Applicants Younger than 18 years of Age:
Portuguese birth certificate;
Portuguese passport or identification card;
1 Recent color passport type photo;
The registration form of minors below the age of 17 must be signed also by parent, tutor or legal guardian, who must present proof of identity plus in the case of legal guardians, proof of having custody.
Note: If registering by mail, signature must be legalized by a local Notary Public.

2.Applicants Older than 18 years of Age:
Valid Portuguese Identification Card
Portuguese passport;
1 Recent color passport type photo;
Registration form signed by applicant if older than 18
Proof of identity and residence: Please present another document with photo and signature, such as a driver’s license, plus proof of employment or school I.D.
Note: If registering by mail, signature must be legalized by a local Notary Public.

3.Other Documents to be Submitted:
Applicants with dual nationality:  copy of citizenship certificate.
Proof of civil status. Changes in marital status/name due to marriage, divorce or widowhood occurred in a foreign country must be endorsed on applicant’s birth certificate or ID card before they may appear in Portuguese documents

4.Applicants Who Do not Hold Valid Portuguese Identification Cards:
Must appear in person at the Consulate and submit documents described on the attached sheet.

Fees
The Consular Registration is free. If registering by mail, please enclose a self-stamped addressed envelope for Certified mail for the return of the documents. Or a money order in the amount of US$5.00 to the order of the Consulate of Portugal, to cover the mailing fees
